I went to Dave & Buster's and sat down at the fireteam raven booth, opened the page on my phone and scanned it. One day later and no badge or progress (despite me beating the game). What did I do wrong?!

Also, the scanners need to be hooked up by D&B - likely they weren't.
Talk to the GM of that location and get them to hook it up.
You might even get a couple free lives from them if you tell them how your experience was effected.

Thanks! There was no light indication on the scanner so it was probobly off, thanks for the tip.
